Health Care Partners of South Carolina (HCPSC) is proud to announce the graduation of more than 40 participants from its award-winning Diabetes Prevention Program, with graduates representing multiple HCPSC locations. While participants came from different communities, they shared one common goal: improving their health and reducing their risk of diabetes.
The Diabetes Prevention Program is a cornerstone of HCPSC’s commitment to preventative, whole-person care. Through evidence-based education, lifestyle coaching, and ongoing support, participants were empowered to make sustainable changes that positively impact their long-term health outcomes. The program’s success has contributed to its recognition as an award-winning initiative within the health care community.
HCPSC continues to see strong engagement and measurable impact across its service areas, reinforcing the value of accessible, community-based prevention programs. This milestone reflects not only the dedication of the participants, but also the collaborative efforts of HCPSC staff and care teams who support patients every step of the way.
