What should I bring with me on the day of my appointment?

You must bring a photo ID with you for your appointment.

Please remember to bring your insurance card(s) with you to your appointment. Bring all cards that may apply.

Please remember to bring all your medications and/or prescriptions to your appointment. Also, notify medical/dental personnel of any changes in your medications since your last appointment.

If you are a new patient or have not been seen within the last year, you must complete a registration form.

All patients registering for services with Health Care Partners of South Carolina who desire a sliding fee discount are required to provide proof of household income. Verification of income is necessary once a year and anytime there is a change in your household’s income level or size.

  • Two most recent pay stubs
  • Most recent tax return
  • Social Security/updated disability statement for the past and current month
  • Retirement statement for the past and current month
  • Unemployment statement for the past and current month

We accept all forms of insurance including Medicaid and Medicare. We also work closely with patients who lack adequate health insurance to help them qualify for our sliding fee discount program.

If I need to see a doctor, why not just go to the emergency room?

The emergency room at a hospital is important for urgent situations at times when other options aren’t available, but Health Care Partners of South Carolina offers something more: With us you have a health care home. This means you have a doctor who knows you and can help manage your whole health. If you need to speak with a provider after hours, our 24-hour answering service (843.248.4700) will be able to assist you.
